Introducing an AI digital tool in to primary care
Engaging older adults with mobility impairment in primary care remains a significant challenge. Many patients have limited experience with digital technology and may be unfamiliar or uncomfortable with artificial intelligence–driven solutions. However, evidence shows that AI-enabled tools have considerable potential to improve early identification of mobility decline and reduce falls risk. The key challenge lies in ensuring meaningful engagement within this population. This presentation describes how our Primary Care Network successfully implemented GaitSmart, an AI-driven gait assessment and personalised exercise programme designed specifically for older adults. By selecting a solution that is accessible, non-invasive, and patient-friendly, and by training a Care Coordinator to deliver the service, we were able to integrate advanced digital technology into routine primary care in a way that felt acceptable and supportive for patients. To date, more than 200 older patients have fully engaged with the programme. Objective gait data demonstrate measurable improvements in mobility, while patients report increased confidence, independence, and quality of life. Importantly, the service has been delivered at minimal cost using existing workforce roles, highlighting its scalability and sustainability. This talk will explore the practical lessons learned from implementing AI-driven mobility assessment in a traditionally hard-to-engage population and will demonstrate how the right technology, delivered by the right team, can transform falls prevention and frailty management in primary care.
